That Forgotten Realms logo was introduced with the Campaign Setting for D&D 3rd edition:

And it doesn't match the maps for the newer set, for 4th edition, so it would be reasonable to conclude the map is from this set.

WotC has what looks like the same map for download:

It is not 100% clear where the map came from on their site, but I got it by searching for the 3rd edition, so...

It doesn't match the map I have in my 3rd edition FR Campaign book.

The map in my book doesn't have that beige border with the logo. It does have the compass in the lower left side accompanied by Copyright 2001 Wizards of the coast. The legend and Faerun are mentioned in the lower right corner where the Great Sea is.

Thanks. A friend emailed me and asked about it. He said that "Attached is a copy of a FR Campaign Setting Map similar to the one that comes with the 3.5 Campaign Setting Hardcover save it has a yellow border at the bottom and is perforated on the left side with a date of 2002. Do you know what FR book or module this was removed from?"
